The Diabetes Care and Coordination Survey

Inside of CenterSarah Derrett, PhD, a 2011-12 New Zealand Harkness Fellow in Health Care Policy and Practice, leads a survey of diabetes care and care coordination among providers and staff with the MidWest Clinicians’ Network (MWCN). This study aims to:

  1. Describe the types of activities and tasks undertaken by health professionals and other staff working in health centers in the care of patients with diabetes;
  2. Describe relationships between staff, team and organizational characteristics and a model of integration;
  3. Describe the scope of practice reported by staff working at MWCN health centers across 10 States of the Midwest.
